Part of a series of MCHIP Quality of Care tools, this document is used to report infrastructure conditions and verify availability and storage conditions of medications, supplies and equipment. It includes a short record review to capture the number of antental care consultations and deliveries at each facility for the last year, based on the health management information system (HMIS) tools (e.g., antenatal care register).
